Two health care groups are coming together to improve national efforts to implement electronic medical records and promote interoperability. The National Transitions of Care Coalition recently joined forces with the Health Information Technology Work Group. One of the coalition’s main goals will be to solve health IT issues regarding transitions of care, like the technical and communication problems that arise when a patient is transferred from a hospital to an assisted living facility.
