VA Healthcare Software Glitches

Earlier this week, the Associated Press reported that health care record software glitches at the Department of Veterans Affairs resulted in glaring errors in some patient medical records. Patients were reportedly given incorrect doses of drugs, and had medical diagnoses switched. No patients were harmed.
