The Veterans Affairs Department’s inspector general released a scathing followup report this week on an earlier alert it issued last year regarding the VA Medical Center in Washington. Among other things, it detailed dirty facilities that were supposed to be sterile, shortages of medical equipment and supplies and a pervasive failure to properly track and order goods and other equipment. The report also said the VA officials at almost every level were made aware of the problems over the course of four years, but failed to fix them. Michael Missal, Veterans Affairs inspector general, joined Jared Serbu on Federal Drive with Tom Temin to discuss the results of the followup.
