Most of the failures came from a lack of voicemail, failing to return phone calls in a timely manner and not properly transferring callers to a sex-assault advocate. Auditors tested both Navy and Marine hotlines.
Despite the 25 percent failure rate, the report actually indicates improvement from the 44 percent failure rate noted in a January audit.
